While we are happy to work with special needs, we do ask that our patients help us by
following some important policies and procedures.
Appointments Urgent appointments are made the day you call; less urgent problems may
be scheduled later. Work-in appointments may have to wait until after scheduled patients
are seen. If you need a work-in appointment, please discuss your needs with the receptionist.
Insurance and Billing SOAC participates in many insurance plans. We are happy to file
claims for you with current insurance and personal information. (Always tell the receptionist
if any of this data changes.) Your insurance co-payment, deductible, co-insurance or non-
covered services are due at the time of your visit. We accept cash, checks, Visa and MasterCard.
For surgical services, our office can help set up a payment plan.
Referrals You are responsible for getting a referral from your primary care physician if it is
required by your insurance. Please let the nurse know of any facility restrictions of your
insurance carrier.
Test Results Test results are returned at varying times. We understand the urgency you feel
to know results, and we promise to contact you as soon as possible. As a precaution, we ask
that you call the office if you have not heard a response in four days.
Surgical Procedures We will work with you to schedule a convenient surgery date. Most
insurance plans require a pre-certification or pre-determination process, which may take some
time. Going through this process assures that your insurance carrier pays all the benefits you
are eligible for.
Medications It is very important that we know all of your current medications. Please bring
a complete list (with dosages) or simply pack up your medication bottles and bring them
with you.
Prescription Refills For medication refills, please call our office during normal business hours.
We will need the patients full name, date of birth, a daytime phone number, the medication
needed (and dosage), the pharmacy name, and pharmacy phone number. Prescriptions will be
called in within 24 hours.
Emergencies Our physicians are on duty after hours and on weekends for emergencies.
Privacy We will not release any medical information without your specific written permission
(including spouses and immediate family members). If the patient is under 18 years old,
medical conditions can be discussed with a parent or legal guardian. This is in compliance with
Federal law.
Records If you require a copy of your records, please allow 10 days.
